Worksite protection in Australia relies heavily on rules and procedures to protect people undertaking rail safety work.

Failure to comply with these represents a major risk.Investigations into worksite protection incidents or near misses often find that someone failed to follow a rule or procedure but they need to go further to identify the root cause for the deviation in work practices.

Systems analysis toolkit

A worksite protection systems analysis toolkit was developed in conjunction with rail transport operators for use by rail operators investigating worksite protection occurrences in NSW.

The toolkit helps operators to gather the data they need to better identify the factors that contribute to human error.

Customising the toolkit

The fault trees within the toolkit are generic and to make them most effective rail operators can modify them to suit their particular operating environment, circumstances and procedures.

The toolkit comprises:

  • Report: Worksite Protection Systems Analysis Toolkit
  • Fault trees (including human factors checklist) for Lookout Working (LW), Controlled Signal Blocking (CSB), Track Work Authority (TWA), Track Occupancy Authority (TOA), Local Possession Authority (LPA) in Excel spreadsheets
  • Instructions for use
