Transport Safety Bureau has released the final report into its investigation
into a derailment of ore train ND575 near Tom Price, Western Australia, on 15
On 15 December 2015, train ND575 was heading south on the Hamersley Railway and
most likely ran over a small horizontal track misalignment, just south of the
Mt Brockman Road railway crossing. It is likely the movement of the train over
the small misalignment led it to increase in size until it was large enough to
cause the derailment of more than 50 ore carriages.
The final report into the incident highlights the need for rail operators to ensure rail defects are detected early, assessed and effectively managed.
